Please note that the Company is responsible for travel fees 2 hours outside the Chicagoland area. Additionally a deposit of 50% is required to hold your date. You will be contacted with a quote after this form is reviewed. Once the 50% deposit has been paid no refunds are provided.You may however be able to choose another date based on availablily.
